Youth & Family Connections is a community based nonprofit service organization founded in 2005. We provide multi-agency services for Weld County youth engaged in high risk behaviors, as well as for their families. Our mission is to create pathways for youth success, along with partnerships with community, civic, and governmental organizations.

There is no cost for our services. Referrals come from a variety of sources, including schools, law enforcement, concerned parents, and many others. When a youth/family visits one of our two office locations, an individual assessment is completed to determine problem areas and help connect them to local services in their immediate community for support.
Youth & Family Connections is a non-profit, community based organization which focuses on prevention and intervention strategies, comprehensive assessments, case management and coordinated access to services. We serve youth under age 18 and their families to offer positive alternatives for growth and support no matter what stage of life they are going through.
